If people can't afford to pay for health care, the employers are to offer health care services. My employer has done this since before I started working on my current job over a decade ago. Granted the "costs" kept increasing every single year, which I really didn't understand.<<
It's really simple. The noted economist W.C.Fields expressed it best nearly 100 years ago: "Never give a sucker an even break".
Your boss, or the government, is gonna pay the bill, right? Then they're a sap. So jack the rates every year, just because, and let them worry how it gets paid for.

It does not need a total overhaul. This is a federal republic where most TREATMENT is organized at the COUNTY LEVEL. Even Obamacare has nothing to do with treatment-- it even makes it harder to open new hospitals-- and just goes on and on and on and on and on about who pays for it. The waste involved in robbing Peter to pay Paul through the Federal Government is so vast that most industrialized nations' GDP would get lost in the abyss.
The core joke of Hitchhiker's Guide to the Galaxy is that of course no civilization would develop personal computers with instant remote database recovery, and then waste this technology to find good drinks.
